Exploring embedding Charting the LifeCourse framework across management, policy, and decision-making activities by working with staff to adopt a person-centered philosophy and mindset. Explore promoting quality reviews to support individual and family
Innovation/Focus Areas:
- Internal capacity building and workshops for systems change, advocacy, self-determination, and engagement
- Integrating CtLC principles on the website and into the Individual Support Plan
- Embracing principles of Charting the LifeCourse within DDS administration and policy development
Success Stories:
- Embedded and adapted Charting the LifeCourse tools within the person-centered planning process for all Individuals receiving waivered supports through the Department of Developmental Services
- Use of the Charting the LifeCourse tools for decision making within the Department of Developmental Services and with partners
- Embedded and use the CT Charting the LifeCouse materials In the CT Youth Leadership Program to support students developing transition plans to adulthood
- Charting the LifeCourse tools are being used by multiple state agencies and organizations to advance person centered planning for Individuals and Families
