Vision for Supporting Families and the CoP in NH:

Introducing and using Charting the LifeCourse framework and principles to develop a vision for a good life across the lifespan.

Innovation/Focus Areas:

  • In collaboration with the Bureau of Developmental Services, the Bureau of Elderly and Adult Services, the Bureau for Child Services, and the Bureau of Mental Health, and the Charting the LifeCourse Steering Committee, the NH CoP Leadership Team is working to develop shared competencies for person centeredness across all the bureaus.

Success Stories:

  • The CoP held a virtual and an in person event to gain additional feedback from stakeholders and professionals regarding what they would like to see from the state’s CtLC work. As a result a focus area of shared competencies has been Identified and work toward reaching that goal has begun.
