Vision for Supporting Families and the CoP in MO:
To cultivate partnerships and network with families, individuals, and stakeholders to increase capacity to influence policies, affect practices, and enhance systems to better assist and support families and individuals across the lifespan.
Have a positive influence on systems and processes with an emphasis on family and person-centered approaches that help to meet support and service needs and reflects the family and individual voice.
Innovation/Focus Areas:
Focus: Professional Development/Capacity Building
- Building Support Coordination competencies and skills utilizing CtLC philosophy, framework, and tools
- Enhancing knowledge and skills through CtLC Ambassador trainings
- Creating a community of CtLC Ambassadors
Focus: Discovery, Planning, Navigation
- Creating trainings for families/individuals utilizing CtLC framework/tools and philosophy
- Enhancing stakeholder involvement and feedback, which includes the voice and perspectives of families/individuals
- Providing information about CtLC through trainings and stakeholder events
- Regional/local entities using CtLC materials with families/individuals for real life planning and peer support
Focus: Front Door
- Established Information Specialists position at DDD front door to help families/individuals navigate systems
- Making direct referrals to resources and peer supports at DDD front door
Focus: System Enhancements
- Incorporating CtLC domains into Missouri Quality Outcomes
Embedding CtLC into Individual Support Plans and planning process
Embedding CtLC framework into quality assurance monitoring practices
Enhanced communication with families, individuals, and stakeholders through e-mail blasts, webinars, guides, social media, videos, and newsletters
Success Stories:
Missouri continues to focus efforts on enhancing DD related systems that focus on processes that affect families and individuals by utilizing the Charting the LifeCourse framework/tools.
Resources and Highlights:
Missouri Quality Outcomes (MOQO): The Missouri Quality Outcomes were developed as a result of listening to people with disabilities, their families, and advocates. The outcomes were designed to encourage personal quality of life outcomes with individual focus on leading a self-determined life; including personal values, choice, health, safety, inclusion and self-advocacy.
